Private In-Home Nursing Care for Families Who Want an Alternative to Skilled Nursing Facilities
GET PRIVATE IN-HOME NURSING CARE
There are more options than the ones handed to you.
The discharge process moves fast, and skilled nursing facilities are almost always the path of least resistance. But the right level of care depends on far more than what’s easiest to arrange before checkout. Nobody gave you a choice. So, we’re giving you one. Some people do need a facility setting, at least temporarily. Others can look at alternatives that allow them to go home with the right clinical support in place. What that looks like is different for every family, and figuring it out together is exactly what we’re here for.
Skilled nursing facilities have their time & place, but here’s what you should know before going all in:
Risk Stats — Navi Nurses
Readmission Rates
CMS data consistently shows patients in a skilled nursing facility are readmitted to the hospital at higher rates than those who receive skilled care at home.
Infection Risk
Institutional care settings carry measurable risk of healthcare-associated infections, including MRSA and C. diff.
Cognitive Decline
Research consistently links institutionalization to accelerated cognitive decline and higher rates of depression in older adults.
Quality Varies Widely
CMS star ratings provide one lens into the quality of care you'll receive but even highly rated facilities operate with staffing ratios that limit one-on-one nursing time.

There’s something comforting about recovering in a familiar environment… like sleeping in your own bed, seeing familiar faces, maintaining routines that still feel like your life instead of adapting to the pace of a facility and their rotating staff…For many people, being home brings a sense of independence and dignity that can make health challenges feel more manageable. But that doesn’t mean being home is always the easiest option without proper support. With private duty nursing, we can make the transition home more manageable for your complex medical needs by bringing experienced clinical care to you — helping you feel safer and recovery feel less overwhelming overall.
Complex discharge instructions that need ongoing nurse-led oversight
Patients whose main barrier to going home is family confidence, not medical acuity
Strong family presence at home
A Facility May Still Be the Right Call for:
24-hour skilled nursing needs that a visiting nurse model cannot safely support
Home environment safety concerns that can't be quickly addressed
Significant cognitive impairment without adequate supervision between visits

How to Pay for Home Care After a Hospital Stay
Navi is a private-pay service and does not bill Medicare or traditional health insurance directly. That said, there are more ways to cover the cost than most families realize, including long-term care insurance, HSA and FSA funds, and veterans’ benefits.
Can my loved one leave a skilled nursing facility to receive care at home?
If your loved one is currently in a skilled nursing facility but would prefer to recover at home, a private duty nurse can provide the clinical support needed to make that transition safe. The first step is a conversation with the facility’s care team and our care coordination team to assess whether in-home nursing is a viable path forward for your specific situation.
How quickly can a Navi nurse start after discharge?
In most cases, we can have a nurse in place within 24 hours of an initial consultation. If a discharge is happening faster than expected, contact us directly and we’ll do everything we can to move at the pace your situation requires.
What if my loved one’s condition is too complex for home care?
It’s a fair question, and we’ll answer it honestly during our initial consultation. For many families, the bar for what qualifies as “too complex for home” is set higher than it needs to be. Our nurses come from acute care backgrounds and are equipped to manage significant clinical complexity in a home setting.
How is private duty nursing different from hiring a caregiver independently?
Professional caregivers provide valuable support with daily tasks such as bathing, dressing, meals, and companionship, but they cannot perform medical tasks. A Navi nurse is a licensed nurse who can perform clinical assessments, manage medications, provide wound care, monitor for early signs of decline, and communicate directly with your care team. If your loved one’s needs require medical tasks only a nurse can perform, a caregiver alone is not the right fit.
Can a Navi nurse work alongside our current home health provider?
Yes. Some families use Medicare home health for the visits it covers and bring in a Navi nurse to fill the gaps such as additional monitoring, more complex tasks, or simply more clinical time. Navi nurses are experienced at working within an existing care team and will communicate across providers to keep everything aligned.
What happens if something changes at home and we need more support?
Care plans are built to evolve. If your loved one’s needs increase — or decrease — we adjust accordingly. Your care coordinator stays in regular contact and is always reachable if something shifts between scheduled visits. Our goal is to stay ahead of changes, not respond to crises.
